Activity: Indoor Social Volleyball Competition and associated activities
Please read this document carefully before agreeing. By checking the box below and participating in the activities, you acknowledge that you have read, understood, and agree to these terms.
1. Acknowledgment of Inherent Risks
I understand and acknowledge that participating in indoor volleyball involves inherent risks, dangers, and hazards. These risks include, but are not limited to:
- Slips, trips, falls, and collisions with the court surface, nets, poles, or other participants.
- Being struck by volleyballs.
- Physical exertion resulting in muscle sprains, strains, fractures, or other bodily injuries.
I acknowledge that I am voluntarily participating in this activity with full knowledge of the potential risks involved.
2. Physical Fitness and Medical Clearance
I declare that I am medically and physically fit to participate in the activities provided by Rhinos Volleyball Club. I agree to disclose any pre-existing medical conditions or injuries to the organizers prior to participating. If I require emergency medical treatment during the activity, I authorize the organizers to secure such treatment on my behalf, and I agree to cover any associated medical or ambulance costs.
3. Release and Indemnity
To the maximum extent permitted by law, I agree to release, hold harmless, and indemnify Rhinos Volleyball Club, its owners, operators, referees, volunteers, and the hired venue (including the school or facility and its staff) from any and all claims, liabilities, demands, or costs arising out of my participation in the competition. This release applies to any injury, loss, or damage to my person or property, except where such injury or damage is caused by the gross negligence or reckless misconduct of the organizers.
4. Code of Conduct and Venue Rules
I agree to abide by the rules of the game, the Rhinos Volleyball Club Code of Conduct, and any specific rules set out by the hired venue. I understand that aggressive behavior, unsportsmanlike conduct, or damage to venue property may result in my immediate removal from the competition without a refund.
